We understand that people must be involved in and central to their care in order to provide high quality healthcare. Your care and treatment are important to us. We will treat you as an equal partner in planning, developing your care plan and monitoring your care and treatment to make sure your needs are met.
Prior to admission, we will carry out a pre-assessment of your needs and a follow up comprehensive nursing assessment. We will work with you and your family to find the best way to provide your care and treatment.
Once agreed, we will record your care and treatment on a digital care plan which will be discussed and agreed you or your representative and communicated to the healthcare team for teamworking.
Our staff will respect your values, take into account your wishes, preferences and expressed needs, working together to make sure there is good communication, making sure you are comfortable and safe, you are provided with emotional support and you have access to care and treatment when you need it.
We believe by putting you and your family at the centre of decision making and working alongside staff you will get best outcome. We will meet with you regularly for reviews and send you questionnaires to obtain feedback about your satisfaction with the care and treatment we provide.
